MTA Extends Deadlines:
Because information from New York State was not distributed until mid-December, when many employees were either taking time off or distracted by year-end work deadlines and holiday planning, the MTA is extending the deadlines for its health insurance opt-out pilot program and the health insurance open enrollment period through Friday, January 13, 2012.

 

Opt-Out Pilot Program
For non-represented employees, the incentive for choosing to opt out is $3,000 if you have family coverage or $1,000 if you have individual coverage.
For represented employees, the opt-out amounts are $550 for an individual plan and $1,100 for a family plan.

 

You can only take advantage of the opt-out if you provide evidence of alternate health insurance. This is a pilot program that the MTA may cancel or modify at its discretion at any time without prior notice.

Open Enrollment
The deadline to choose an alternate health care provider has also been extended to January 13, 2012. The annual open enrollment for the New York State Health Insurance Program (NYSHIP) applies to all MTA full-time employees.

 

You can choose coverage from the Empire Plan or an available health maintenance organization (HMO). Please refer to the NYSHIP Health Insurance Choices for 2012 booklet.

Collecting Facts For a Claim or Grievance
The five plus two W's
 
1. Who was involved?
     (a) Name and position?
     (b) Covered by agreement?
 
2. What happened to make it a claim or grievance?
 
3. Where did it happen?
     (a) Did it happen more than once?
     (b) If so. did it happen in more than one period?
 
4. When did it happen?
     (a) Day of the week?
     (b)Date?
     (c) Time of day?
     (d) During or outside the claimant(s) working hours?
 
5. Why did it happen? (Not why it is a grievance.)
     (a) Caution ! Is this a "fact" or "opinion", or a mixture of both"? (Separate
           the two.)
     (b) If you cannot get enough facts to prove your opinion as to why it
           happened, then what are the facts that have led you to your opinion?
